Skilled Nursing Facility Travel assignment supported by Healthforce Healthcare LLC Responsibilities Provide direct patient care to residents under supervision of an RN, including assessment and monitoring. Administer prescribed medications and document administration accurately in resident records. Monitor vital signs, assess for changes in condition, and report findings promptly. Collaborate with the interdisciplinary care team to implement and adjust care plans. Respond to emergencies, apply basic nursing interventions, and escalate as needed. Maintain accurate, up-to-date documentation and follow infection control and safety protocols. Requirements Availability for night shift (7:00 PM-7:30 AM) on a 3x12 schedule. Willingness to participate in a travel assignment lasting approximately 8 weeks. Ability to work effectively within a skilled nursing facility environment and team-based care. Strong communication and teamwork skills to coordinate resident care. Capability to complete credentialing or onboarding steps required for assignment.
