Perform and document ordered wound care and treatments in accordance with physician orders, facility policies, and applicable regulations. Provide treatment for acute and chronic wounds, ostomies, stomas, skin tears, pressure injuries, and skin breakdown related to urinary or fecal incontinence . Monitor residents for changes in skin condition and promptly communicate concerns to appropriate nursing staff and providers. Work closely with licensed nursing staff to identify and address risks such as skin breakdown, contractures, weight loss, and other conditions that may affect residents' functional status. Coordinate treatments with residents' daily schedules and other departments to promote continuity of care. Assist with development, implementation, and follow-up of resident care plans related to wounds, skin integrity, and treatments. Maintain accurate and timely documentation in accordance with facility policies and procedures. Communicate with physicians, providers, families, nursing staff, therapy, dietary, and other members of the interdisciplinary team as needed. Participate in staff meetings, in-services, quality assurance activities, and wound prevention initiatives. Provide a multidisciplinary approach to wound management and treatment to promote healing and maintain the highest possible level of resident care. #IND123M
