In this vital role, you will provide high-quality nursing care to patients across diverse settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient well-being through comprehensive assessments, medication administration, and personalized care plans. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a collaborative and supportive environment. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately, including IV infusions and catheterizations, following strict protocols and safety standards. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Manage patient care activities within EMR/EHR systems, documenting all interventions clearly and promptly while maintaining HIPAA compliance. Support patients with activities of daily living, including mobility assistance using equipment like Hoyer lifts and providing specialized dementia or Alzheimer's care. Collaborate with physicians and interdisciplinary teams to develop and implement individualized treatment plans. Educate patients and their families on health management strategies, medication usage, and post-discharge care instructions. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or region. Proven experience working with diverse patient populations across settings such as long-term care, hospitals (including ICU or trauma centers), outpatient clinics, or home health environments. Strong knowledge of medical terminology, anatomy, physiology, and nursing best practices. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation. Experience in critical care areas like ICU or emergency medicine is highly desirable. Ability to perform clinical procedures including IV insertion, venipuncture, catheter care, suturing assistance, and first aid techniques. Excellent communication skills for caregiver interaction and patient education while adhering to clinical confidentiality policies.
