Qualifications LPN Full Job Description Overview:

Responsibilities:
Obtain and document vital signs such as blood pressure, temperature, pulse, and respiratory rate. Assist physicians and RNs with examinations, treatments, and care plans. Administer medications and treatments as prescribed, and injections. Prepare patients for exams, gather medical histories, and ensure exam rooms are clean and stocked. Document patient information accurately in electronic health records (EHR). Educate patients on medications, chronic disease management, and preventive care. Monitor patient condition and report changes to supervising nurses or providers. Assess patient symptoms via phone or in‑person to determine urgency. Prioritize care using established triage protocols and assign urgency levels. Provide medical advice within LPN scope, including self‑care instructions for non‑urgent issues. Document all triage interactions and recommendations in the EHR. Coordinate referrals to physicians, specialists, urgent care, or emergency departments. Recognize changes in patient condition and escalate complex cases to RNs or physicians. Educate patients on medication management and preventive health measures. Assist with INR and device clinic, process blood samples for INR checks Coordinate necessary care for device patients under EP supervision Completes all mandatory department, educational and hospital requirements Adheres to current Infection Control and Safety Standards Regular and prompt attendance Ability to work schedule as defined and overtime as required Related duties as assigned
Additional Information:
Employees must be in a regular position, working 20 hours or more per week (.50 FTE or greater) to qualify for benefits.
Qualifications:
Graduation from an accredited school of practical nursing meeting the minimum standards set by the State of Alabama Required LPN
  • Licensed Practical Nurse
  • State Licensure or current Multi-State License in accordance with the Nurse Licensure Compact (NLC) for Alabama Required Comparable combination of education and experience may substitute for the above requirements.
