Licensed Practical Nurse (LPN) Opportunity We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our exceptional healthcare team. In this rewarding role, you will provide direct nursing care to residents, support individualized rehabilitation plans, and collaborate with Registered Nurses, physicians, and other healthcare professionals to ensure outstanding clinical outcomes. As an LPN at Opp Health & Rehab, you will play a key role in enhancing the well-being of our residents while working in a team-oriented environment that values professionalism, compassion, and excellence in care. Responsibilities Administer medications and treatments as prescribed by physicians. Monitor and document residents' health status, including vital signs, and report changes to the supervising RN or physician. Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed. Collaborate with the rehabilitation team to support residents' recovery goals, including physical, occupational, and speech therapy plans. Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations. Provide emotional support and education to residents and their families. Ensure a safe and clean environment, adhering to infection control protocols. Qualifications Current and valid LPN license Strong clinical skills, including medication administration and wound care. Excellent communication and interpersonal skills. Ability to work effectively in a team-oriented environment. Compassionate, patient-focused approach to care. BLS/CPR certification required.
