As a Home Health Licensed Practical Nurse (LPN), you will play a key role in supporting patient independence and recovery, while maintaining effective collaboration with families and the healthcare team.

Schedule:

Monday Friday, 8:00 AM 5:00 PM Occasional flexibility required for patient needs

Key Responsibilities:

Conduct skilled nursing visits, assessing and implementing individualized care plans Administer medications, perform wound care, and manage IV therapies as prescribed Observe and document changes in patient conditions; report promptly to RN Case Managers or physicians Educate patients and caregivers regarding disease management, medication, and self-care practices Ensure all charting and care documentation is accurately completed in Home Care Home Base (HCHB) Collaborate with the interdisciplinary team to optimize care outcomes and maintain compliance Adhere strictly to infection control standards to guarantee safety Participate in case conferences, ongoing education, and quality initiatives

Qualifications:

Active, unrestricted LPN license in the state of practice Minimum one year of home health or community-based care experience Proficiency with Home Care Home Base (HCHB) Valid drivers license, reliable transportation, and current auto insurance Current Basic Life Support (BLS) certification Effective clinical judgment, time management, and communication skills Ability to work autonomously and organize a field-based caseload

Preferred Experience:

Background in Medicare-certified home health agencies Thorough understanding of home health compliance and documentation standards Strong interpersonal skills and devotion to patient-centered care
