Responsibilities Perform comprehensive patient assessments, including vital signs, intake, and physical examinations to determine immediate needs. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration. Document all patient interactions thoroughly using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems like PCC, ensuring compliance with HIPAA regulations. Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans for patients with complex conditions such as dementia, Alzheimer's disease. Experience Valid LPN license with current state licensure required. Demonstrated proficiency with EMR/EHR systems like PCC. Background working with individuals with disabilities or those requiring specialized memory or dementia care is desirable. Strong understanding of physiology, anatomy, infection control practices, and emergency response protocols such as basic life support (BLS).
