Job Description

Home Health Licensed Practical Nurse (LPN) Eureka Springs, AR Area We are seeking an experienced Home Health LPN to join a dedicated patient care team serving the Eureka Springs area. This role offers a guaranteed 32 hours per week, mileage reimbursement between patient visits, and the opportunity to make a meaningful impact in patients' homes. Schedule Monday-Friday, 8:00 AM - 4:30 PM On-call requirement: 1 weekend per month Potential holiday coverage as needed Compensation $32.00/hour Mileage reimbursement between patient visits Key Responsibilities Provide skilled nursing care in accordance with physician orders and established care plans. Monitor patient conditions and communicate changes to the supervising RN and healthcare team. Coordinate patient care with physicians, families, and agency staff. Complete accurate and timely documentation, including medication reconciliation and required clinical documentation. Maintain compliance with infection control standards, agency policies, and payer requirements. Participate in case conferences, performance improvement initiatives, and episode management processes. Support efforts to reduce hospitalizations and improve patient outcomes. Participate in weekend on-call rotation and provide flexible scheduling support when needed. Required Qualifications Current Licensed Practical Nurse (LPN) license Recent Home Health experience required Current CPR certification Valid driver's license, reliable transportation, and ability to travel throughout the assigned territory Preferred Qualifications Experience with Homecare Homebase (HCHB) Strong communication, organization, and documentation skills Ability to work independently in a home health setting If you are an experienced Home Health LPN looking to join a patient-focused organization with a strong commitment to quality care, we encourage you to apply today.
