Overview LPN - Van Buren Health & Rehab Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) working overnight shifts from 11 p.m. to 7 a.m., Monday through Friday, with weekend double shifts and Baylor pay. Duties Communicate with families and residents Provide supervision to staff performing ADL's DIrect communication with the MD if needed Follow up and document on any incidents/accidents Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration. Document all patient interactions meticulously within Electronic Health Record (EHR) PointClickCare, ensuring compliance. Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care strategies for diverse patient populations.
