Position Summary The Behavioral Health Clinic Licensed Practical Nurse (LPN) provides safe, compassionate, and clinically appropriate nursing care to individuals experiencing mental health challenges, behavioral health concerns, and/or substance use and withdrawal symptoms. Working under the supervision of a Registered Nurse (RN), physician, or other authorized healthcare provider, the LPN supports the delivery of medical and behavioral health services within the clinic setting. The LPN is responsible for patient monitoring, basic nursing care, behavioral observation, group support, clinic coordination, documentation, and collaboration with the interdisciplinary treatment team. The position requires strong communication skills, sound clinical judgment within the LPN's scope of practice, professionalism, and the ability to maintain a safe, structured, and therapeutic environment. Essential Job Duties and Responsibilities Patient Monitoring and Direct Clinical Care

• Monitor and document patient vital signs, including blood pressure, temperature, pulse, respirations, and other clinical measurements as indicated.
• Observe and document changes in patients' physical and behavioral presentation, mental status, and overall functioning.
• Monitor patients for signs and symptoms of substance withdrawal and promptly communicate concerns to the appropriate medical provider.
• Provide basic nursing care, comfort measures, dressing changes, wound care, and other services within the LPN's scope of practice and as directed by the supervising provider.
• Assist with medication-related duties as permitted by state regulations, organizational policy, and the LPN's scope of practice.
• Identify and promptly report significant changes in patient condition, safety concerns, medical concerns, or escalating behavioral symptoms to the supervising RN, physician, or appropriate member of the treatment team.
• Participate in emergency and crisis response procedures in accordance with organizational policies and approved safety protocols. Behavioral Health and Milieu Support
• Assist with maintaining a safe, structured, supportive, and therapeutic clinic environment.
• Assist with planning, facilitating, and/or co-facilitating health education, psychoeducational, wellness, and community-based groups as assigned.
• Encourage patients to utilize appropriate coping skills, grounding techniques, emotional regulation strategies, and other treatment-supportive interventions.
• Provide appropriate support to patients experiencing emotional distress or behavioral escalation.
• Assist the interdisciplinary team with crisis prevention and de-escalation using approved, trauma-informed techniques.
• Maintain appropriate professional boundaries while interacting with patients, families, and members of the treatment team. Clinic Operations and Care Coordination
• Assist with coordinating patient appointments, provider schedules, laboratory services, diagnostic testing, and other clinical services.
• Coordinate and track routine testing, including urine drug screens, as assigned.
• Support efficient patient flow and daily clinic operations.
• Communicate scheduling changes, clinical needs, and other relevant information to appropriate staff.
• Assist with coordination of care between medical providers and members of the behavioral health treatment team.
• Maintain awareness of daily clinic activities and assist with addressing operational needs as they arise. Communication and Documentation
• Establish professional, respectful, and therapeutic relationships with patients.
• Listen actively to patient concerns and provide appropriate, non-judgmental support within the LPN's role.
• Communicate relevant clinical and behavioral observations to the appropriate members of the treatment team.
• Accurately and timely document vital signs, patient observations, interventions, group participation, patient responses, and other clinical information in the Electronic Health Record (EHR).
• Maintain accurate, objective, and confidential clinical documentation in accordance with organizational policies and applicable laws and regulations.
• Protect patient privacy and confidentiality at all times. Professional Expectations and Competencies
• Practice consistently within the applicable state-defined scope of practice for Licensed Practical Nurses and organizational policies.
• Demonstrate sound clinical judgment and recognize when consultation or escalation to an RN, physician, or other qualified provider is necessary.

• Work collaboratively with RNs, physicians, psychiatrists, therapists, social workers, behavioral health technicians, case managers, education staff, and other members of the interdisciplinary treatment team. Minimum Qualifications and Job Requirements Education
• Graduation from an accredited Practical Nursing program.
• Current and valid LPN education credentials appropriate for licensure in the state of practice. Licensure
• Active, unrestricted Licensed Practical Nurse (LPN) license in the applicable state.
• License must remain in good standing throughout employment. Certifications
• Current Basic Life Support (BLS) certification required.
• Crisis Prevention Intervention (CPI), Nonviolent Crisis Intervention, or comparable behavioral de-escalation training preferred. Experience
• Previous experience in behavioral health, mental health, substance use treatment, psychiatric care, or a related healthcare setting strongly preferred.
• Experience working with individuals experiencing behavioral or emotional challenges preferred.
• Experience with group facilitation, patient education, clinical scheduling, or care coordination is preferred. Physical and Environmental Requirements
• Ability to remain on feet for extended periods and perform routine clinical duties.
• Ability to assist with patient safety and emergency response procedures as required by organizational policy and within the employee's training and scope of practice.
• Ability to maintain professional composure and respond appropriately in stressful or emergency situations.
• Ability to occasionally lift, carry, push, or assist with items weighing up to 50 pounds , as required by job duties and organizational needs.
• Ability and willingness to engage in light local travel on an occasional basis for job-related duties, meetings, training, community-based services, or other organizational needs.
• Ability to work effectively in a behavioral health environment where patients may experience emotional distress, behavioral escalation, or medical concerns.
