Position Summary:

We are seeking an experienced Licensed Practical Nurse (LPN) to provide nursing care to residents at the Veterans' Home in Flagstaff. The LPN will administer medications, perform treatments, monitor resident conditions, maintain clinical documentation, and work collaboratively with residents, families, and the interdisciplinary care team.

6:00 AM - 6:00 PM Full-time or PRN availability Block scheduling with rotating shifts Two consecutive days off Every other weekend 40 hours/week for full-time schedule

Local Candidates Only Key Responsibilities:

Administer medications and perform treatments as ordered by physicians and within the scope of the LPN license. Administer medications through various routes, including oral, enteral, parenteral, and suppository.

Perform treatments such as:

Sterile/aseptic wound care Postural drainage and percussion Tracheostomy care Gastrostomy and jejunostomy care Catheter care Observe residents and report the effectiveness of medications and treatments. Monitor residents' medical, social, and functional needs. Receive and provide shift reports. Communicate effectively with residents, families, and interdisciplinary staff. Complete initial nursing assessments for newly admitted residents. Assist with MDS assessments, quarterly reviews, and care plans . Maintain accurate clinical and administrative documentation. Participate in Quality Assurance and Performance Improvement (QAPI) activities. Attend and participate in staff meetings. Ensure adequate supplies and medications are available.

Required Knowledge & Skills:

Knowledge of gerontological/geriatric nursing principles and practices. Understanding of the effects of the aging process. Knowledge of team nursing practices. Knowledge of treatment plans and patient/resident care procedures. Knowledge of pharmacology and medication administration. Understanding of federal, state, and agency regulations governing practical nursing. Ability to observe, assess, and report changes in resident condition. Ability to assist with development and implementation of patient care plans. Strong oral and written communication skills. Strong interpersonal and teamwork skills. Ability to respond appropriately to medical emergencies. Ability to evaluate situations, make independent decisions, and follow up on resident health concerns. Ability to establish and maintain effective working relationships.

Best Candidate Profile:

An Arizona-licensed LPN with long-term care, skilled nursing, geriatric, or veterans' care experience, particularly someone comfortable with medication administration, wound care, trach/G-tube/catheter care, assessments, MDS, care planning, and rotating 12-hour day shifts. #HC1

Pay:

$44.45 per hour Expected hours: 40.0 per week
