As a Health Services Coordinator (LPN or RN), you'll play a hands-on role in improving the lives of our residents by: Conducting comprehensive resident assessments prior to move-in, re-entry, and on an ongoing basis to ensure all physical, mental, and social needs are met. Assisting with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness. Establishing and fostering positive relationships with residents, families, associates, home health, hospice, and other external partners. Coordinating and ensuring proper health-related training and education for the caregiver team, conducting skill observations to optimize resident care delivery. Providing direct resident assistance as needed, including personal care activities and administering medication in accordance with physician orders.What We're Looking For:
An active Nursing license with the state. A current CPR/BLS certification. 1+ years of experience in a senior living or related industry supervisory or leadership role. 1+ years of experience providing care to cognitive and memory-care residents or patients (preferred). Valid driver's license and acceptable driving record; ability to provide resident transportation and/or serve alcoholic beverages at community events. The ability to assist residents with limited mobility, lift/push/pull up to 50lbs. Excellent verbal and written communication skills, with the ability to effectively collaborate with diverse groups, navigate challenging situations, and make sound decisions. Strong technical skills (e.g., Word, Excel, PowerPoint) and the ability to maintain confidential and privileged information (including HIPAA, residents, and associates). Why Choose Orchard Pointe?
