Key Responsibilities:

Skilled Charting & Documentation:

Perform daily skilled charting for residents, documenting assessments, treatments, medications, and changes in condition accurately and in a timely manner. Ensure all records comply with facility policies, regulatory requirements, and best practice standards.

Resident Assessments:

Conduct regular assessments of residents' physical, mental, and emotional health. Monitor vital signs, medical history, and response to treatments, and report findings to the Director of Nursing or attending physician.

Medication Administration:

Administer prescribed medications and treatments in accordance with facility protocols and physicians' orders. Ensure medication cart is organized, stocked, and cleaned daily, adhering to safety and infection control practices.

Admission Process:

Assist with the admission process for new residents, including reviewing medical history, conducting initial assessments, and ensuring that all documentation is completed accurately and promptly.

Infection Control:

Adhere to infection control policies and practices, ensuring a safe and sterile environment for residents and staff. Participate in ongoing infection prevention and control initiatives.

Delegation & CNA Support:

Provide guidance, support, and delegation of tasks to CNA's, ensuring efficient and quality care for residents. Supervise CNAs in the daily delivery of personal care and other duties. Communication with

Providers:

Maintain open communication with physicians, nurse practitioners, and other healthcare providers to ensure continuity of care and address any changes in residents' health conditions. Relay important updates regarding patient condition and treatment needs.

General Nursing Duties:

Assist with day-to-day nursing duties and provide direct patient care as needed. Ensure that residents' safety, comfort, and needs are met, addressing concerns promptly and professionally. Other Duties as

Take on additional tasks as needed to support the team and provide the highest quality care to residents.

Qualifications:

Education:

Must be a graduate of an accredited practical nursing program. Current, active LPN license in the state of Arizona.

Experience:

Previous experience in a skilled nursing facility or long-term care setting preferred. Experience with medication administration and charting is a must. Experience working with residents with chronic conditions or rehabilitation needs is a plus. Familiar with PCC.

Skills & Abilities:

Excellent communication and interpersonal skills, with the ability to work effectively in a team environment. Strong organizational skills, with the ability to manage multiple tasks and prioritize effectively. Knowledge of infection control practices and the ability to adhere to safety protocols.
