Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans. Administer medications, injections—including IV infusions—and treatments following established protocols while ensuring patient safety and comfort. Manage advanced procedures such as catheterization, tube feeding, wound suturing, and ventilator care with precision and aseptic technique. Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic or Cerner, maintaining strict compliance with HIPAA and confidentiality policies. Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and follow-up care tailored to individual needs. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral health support. Respond promptly to urgent situations such as acute pain episodes or emergency triage scenarios in hospital or outpatient settings. Skills Proven experience working with diverse patient populations including geriatrics, pediatrics, neonatal units, or individuals with developmental disabilities. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Skilled in using EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation. Competent in performing invasive procedures like venipuncture, phlebotomy, sterile processing, and specimen collection/processing. Familiarity with advanced clinical practices including ICU or Level I Trauma Center experience; critical care experience is highly valued. Ability to handle complex cases involving pain management, infectious disease care, hospice services, or post-acute care environments. Excellent caregiver communication skills to effectively educate patients and families about treatment plans and health coaching strategies.
