Pride Health is hiring a LPN/LVN - Home Health to support our client's healthcare facility in Phoenix, AZ.

• Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Provide subsequent visits for home health patients.
  
• Provide wound care, catheter changes, compression wraps, trach care, wound vac management, medication management and education.
  
• Provide disease education, UA, NG tube care, nephrostomy tube care, and trach care.

Requirements:

• License(s): Active LPN license
  
• Certification(s): BLS/CPR
  
• Years of experience: 1+
