Job Summary At MTN Home Health , we believe nursing should feel intentional, unrushed, and deeply human . We are a nurse-led home health agency seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide thoughtful, hands-on care to patients in their homes. This role is ideal for an LPN who values relationship-based care , enjoys working closely with Registered Nurses, and understands that good nursing prevents problems before they become emergencies . This is a paid position offering flexibility, strong clinical support, and meaningful patient relationships. What You'll Do Provide skilled nursing care in the home under the supervision of a Registered Nurse Perform focused nursing assessments and report changes in patient condition promptly Provide wound care, catheter care, ostomy care, and basic treatments per plan of care Reinforce patient and caregiver education provided by the RN Monitor vital signs and patient response to treatments Maintain accurate, timely documentation in the electronic health record Communicate clearly with RNs, providers, patients, and families Support a calm, safe, and respectful care environment in the home Who This Role Is For This role is for an LPN who: Values quality over quantity Is comfortable working independently but appreciates strong RN support Takes pride in being observant, thorough, and reliable Understands that home health is about continuity, trust, and prevention Wants to be part of a team that truly respects nursing judgment Qualifications Active Licensed Practical Nurse (LPN) license (state-required) Home health experience preferred, but not required Strong clinical skills within LPN scope of practice Excellent communication and documentation skills Ability to work independently in patients' homes Dependable transportation and willingness to travel locally Why Work With Us Nurse-led leadership that understands nursing work Thoughtful patient scheduling (no rushed visits) Strong RN collaboration and support Flexible scheduling options A team culture built on trust, respect, and compassion At MTN Home Health, LPNs are valued as essential members of the care team , not task-doers.
