On the Road - West Valley, Arizona (Avondale, Buckeye, El Mirage, Glendale, Goodyear, Litchfield Park, Peoria, Sun City & Sun City West, Surprise, Tolleson, Wickenburg, and Youngtown) We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse. In this role, you will deliver high-quality, patient-centered care including wound care, medication administration, monitoring patient conditions, and completing accurate and timely documentation in accordance with care plans and agency policies. Pay are per visit.

Responsibilities:

Provide skilled nursing care in patients' homes Perform wound care and medication administration Monitor and report changes in patient conditions Complete required clinical documentation accurately and on time Collaborate with the RN and care team to ensure quality patient outcomes

Qualifications:

Active LPN License (required) Current BLS Certification Current CPR Certification Minimum 1 year of nursing experience Home health experience preferred Join our team!
