36 Hours Per Week Job Overview Seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide high-quality nursing care within a Skilled Nursing Facility (SNF) environment. The LVN will work collaboratively with physicians, nurses, residents, families, and interdisciplinary departments to ensure safe, individualized, and patient-centered care. Required Qualifications Active California LVN License Current BLS Certification Minimum 1 year of nursing experience Strong clinical assessment skills Excellent communication and interpersonal abilities Ability to work effectively in a team environment Key Responsibilities Resident Care Perform comprehensive resident assessments and report changes in condition. Develop and implement individualized care plans. Administer medications safely and accurately according to physician orders. Monitor treatment effectiveness and document resident responses. Provide direct patient care while maintaining resident dignity and independence. Clinical Duties Medication Administration IV Starts Blood Draws EKG Procedures Specimen Collection Wound Care Vital Signs Monitoring Emergency Response and Resuscitation Support Documentation & Communication Maintain accurate and timely nursing documentation. Communicate resident status changes to physicians and nursing leadership. Participate in shift reports and interdisciplinary care coordination. Provide resident and family education regarding treatments and care plans. Safety & Compliance Follow infection prevention and safety standards. Maintain regulatory compliance and nursing standards of practice. Ensure confidentiality of resident information. Support quality improvement and patient safety initiatives.
