Berkeley, CA Job Details Per diem $40 - $42 an hour 22 hours ago Qualifications Skilled nursing facility experience LVN Wound care Post-acute care experience Full Job Description Job Summary Experienced Licensed Vocational Nurse (LVN) with a strong background providing direct care in a 77-bed skilled nursing facility. Skilled in medication administration, wound care, patient assessment, care-plan implementation, clinical documentation, and monitoring changes in residents' conditions. Proven ability to supervise nursing assistants, coordinate with interdisciplinary teams, communicate with residents and families, and maintain compliance with safety, infection-control, and regulatory standards. Compassionate, organized, and effective in managing multiple residents in a fast-paced long-term and post-acute care environment. Responsibilities Provide direct nursing care to residents according to individualized care plans and physician/provider orders. Perform and document comprehensive nursing assessments, including changes in condition. Administer medications and treatments safely and accurately according to physician orders and facility policies. Monitor residents for adverse medication reactions, infections, falls, skin changes, and other clinical concerns. Obtain and monitor vital signs, blood glucose levels, weights, oxygen saturation, and other ordered measurements. Recognize changes in residents' condition and promptly notify the RN, physician/provider, and responsible party as appropriate. Supervise and provide direction to CNAs regarding resident care and safety.

Pay:

$40.00 - $42.00 per hour
