In this vital role, you will provide high-quality nursing care to diverse patient populations across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and personalized care plans. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with complex health needs while working within a collaborative and energetic team. Duties Perform detailed patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders while adhering to HIPAA and clinical confidentiality policies. Manage IV infusions and catheter care with precision, ensuring patient safety during complex procedures like gastric lavage or tube feeding. Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures to support comprehensive patient care. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral management techniques. Support acute pain management and airway management for patients in emergency room (ER), ICU, or trauma center settings such as Level I or Level II trauma centers. Collaborate with multidisciplinary teams on case management plans for patients with infectious diseases, geriatrics needs, or post-acute care requirements. Educate patients and caregivers on health coaching topics like medication administration, nutrition, wound care, and mobility assistance using tools like Hoyer lifts or DME (Durable Medical Equipment). Qualifications Valid LVN license in good standing with the state licensing board. Proven experience working in diverse healthcare environments including hospitals (ER, ICU, PICU), skilled nursing facilities, hospice care, or home & community care settings. Critical care experience preferred; familiarity with Level I/II trauma centers or emergency medicine is highly desirable. Proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks; experience with ICD coding (ICD-9/10) is advantageous. Strong knowledge of physiology, anatomy, medical terminology, infection control protocols, and long-term care regulations. Experience working with individuals with disabilities or developmental delays; dementia or Alzheimer's care experience is a plus. Ability to perform venipuncture (phlebotomy), specimen collection/processing, and sterile techniques confidently. Excellent communication skills for effective caregiver communication and patient education across diverse populations including geriatrics and pediatrics. Physical ability to stand for extended periods and assist patients with mobility aids such as Hoyer lifts; CPR certification required.
