In this role, you will provide high-quality, patient-centered care in the comfort of patients' homes while working alongside a supportive interdisciplinary team. This is an excellent opportunity for LVNs who enjoy flexible scheduling, and building meaningful relationships with patient What we offer Competitive per-visit pay $55 per visit Flexible Scheduling Supportive Leadership Duties Perform thorough patient observations, including vital signs, medication administration, and assessment of health status Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document medical records accurately and maintain confidentiality per HIPAA regulations. Conduct catheter care, wound management, and other specialized procedures with precision and adherence to safety protocols. Support patients with medication management, including timely administration and monitoring for adverse reactions. Maintain compliance with home healthcare regulations and standards while delivering exceptional home & community care assistance. Collaborate with the interdisciplinary team to ensure optimal patient outcomes Experience Proven experience working in assisted living facilities, nursing homes, or home healthcare environments. ( Home Health Preferred ) Familiarity with medical terminology and documentation practices within home health agency settings. Strong understanding of home health care assistance regulations and best practices for patient safety and confidentiality. Ability to perform heavy lifting when necessary while maintaining safety protocols for both patients and staff.
