We are seeking a dedicated and highly skilled Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience across various medical settings including long-term care, assisted living, and acute care environments. As an LVN, you will play a vital role in delivering compassionate nursing services, supporting patient recovery, and ensuring compliance with healthcare regulations. Provide direct patient care by monitoring vital signs, administering medications, and performing routine assessments in accordance with physician orders and nursing protocols. Assist with complex procedures such as IV infusion, catheterization, wound care, and medication administration while maintaining sterile techniques. Document patient information accurately using Electronic Health Records (EHR) systems such as Epic, Cerner, or Athenahealth to ensure precise medical records management. Collaborate with interdisciplinary teams including physicians, therapists, and caregivers to develop and implement individualized care plans tailored to each patient's needs. Support patients with activities of daily living (ADLs), including mobility assistance, tube feeding, and personal hygiene tasks within the scope of LVN practice. Participate in triage activities by assessing patient symptoms and prioritizing urgent cases for immediate attention or escalation. Ensure compliance with HIPAA regulations and maintain strict confidentiality of all patient information while adhering to infection control protocols. Engage in ongoing education related to geriatrics, dementia care, hospice services, and specialized areas such as Alzheimer's or memory care to enhance patient outcomes. Requirements Valid Licensed Vocational Nurse (LVN) license issued by the state licensing board. Proven experience working in diverse healthcare settings such as hospitals, nursing homes, skilled nursing facilities, or home health environments. Knowledge of medical terminology, physiology, anatomy, and ICD coding (ICD-9/10) for accurate documentation and billing processes. Hands-on experience with EMR/EHR systems including Epic, Cerner, or eClinicalWorks; familiarity with medical imaging or laboratory specimen collection is a plus. Strong clinical skills in patient assessment, vital signs monitoring, medication administration (including injections), and basic life support (BLS/CPR). Experience working with geriatric patients or individuals with disabilities; familiarity with dementia care and behavioral health considerations is highly desirable. Ability to communicate effectively with patients of all ages and backgrounds while demonstrating compassion and professionalism. Knowledge of long-term care regulations, infection control procedures, and acute pain management practices within fast-paced environments such as ICU or Level I trauma centers is advantageous.
