$70,000 - $75,000 per year About the Opportunity Quantum Care Place of El Dorado Hills is seeking an LVN Resident Care Coordinator to coordinate resident care, support individualized service plans, and serve as a trusted liaison for residents, families, healthcare providers, and team members. Responsibilities Coordinate and oversee implementation of individualized resident care plans and service-plan-related interventions. Monitor resident conditions, identify changes promptly, and initiate or communicate appropriate interventions. Conduct rounds and monitor care delivery, resident satisfaction, documentation, and staff follow-through. Serve as a liaison among residents, families, nursing staff, care staff, physicians, and external healthcare providers. Participate in care conferences, family meetings, admissions, discharges, transfers, audits, quality-improvement activities, and process-improvement initiatives. Review and update records, care plans, and documentation for accuracy, timeliness, and compliance. Educate residents, families, and staff on care processes, medications, protocols, and resident needs. Assist with medication administration, treatments, direct care, and other nursing responsibilities within scope and assignment. Qualifications Current, active, and unencumbered California LVN license. Assisted living, senior living, long-term care, or related healthcare experience preferred. Strong knowledge of nursing care, care planning, coordination, documentation, and resident-centered service. Current CPR certification and any additional credentials required for the assignment. Strong interpersonal, organizational, leadership, problem-solving, and communication skills. Ability to stand and walk throughout the community, assist residents with mobility, and lift, push, pull, or move up to 50 pounds as required. $70,000.00 - $75,000.00 per year
