Full Job Description Description:

The Per Diem LVN provides professional nursing care under supervision of RN for assigned patients across the life span (perinatal, pediatrics, and adults) in patients' homes or facilities according to established standards and practice. Participates as a team member of the clinical interdisciplinary team to ensure appropriate resources to all internal/external customers. Follows The Elizabeth Hospice rules, policies, procedures, applicable laws and standards addressed in the Compliance Program. Responsible for carrying out the mission, vision, values and quality commitment of The Elizabeth Hospice. The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We serve all of San Diego County and south Riverside County with multiple hospice office locations.

Salary:

$36 - $46 hourly. Compensation is determined by years of licensure.

Work Schedule:

Days and hours vary. Candidate must provide at least 6 shifts of availability per month with at least 2 shifts on a weekend day. Workdays will vary but work hours are 4:00pm-12:30am.

Work Location:

South Riverside and San Diego, CA Travel required for patient visits. Occasional travel for educational purposes. LVNs will receive additional shift differential pay when at least 2 hours of a shift are worked between 7:00pm-7:00am.

Requirements:

Responsibilities:

1. Assists RN in analyzing situation, anticipating problem areas and taking appropriate course of action. 2. Evaluates the response of patient, relative to the expected outcomes in a safe and timely manner. 3. Performs licensed nurse skills as stated in basic skills checklist, policies and procedures, standards of care, critical competencies based on scope of practice. 4. Adheres to the direction provided by the RN on the plan of care for the patient. 5.Responsible for delivering services under the supervision of the and in collaboration with the RN. 6. Under the supervision of the RN, implements and documents teaching plan based on: mutually assessed needs of patient and family, in a timely manner, teaching strategy suitable for the learning ability and cultural background of the patient and family. 7. Documents completely all care provided to patient including telephone interaction with other staff members. Charting standards are followed. Maintains timely patient documentation using "point of care" electronic charting. 8.Enters or retrieves data from established computer files. 9. Orients, instructs and trains assigned personnel. 10. Performs Phlebotomy as needed. 11. Provides support to agency in completion of assigned audit and other office duties as assigned. 12. Adhering to the E-way standards is a requirement in performing all job duties.

Qualifications:

1. Vocational/Specialized Training. 2. Minimum of 12 months experience as an LVN with preferred experience of 1-2 years in acute care setting, home health or hospice. 3. Current California LVN license. 4. Ability to answer phones, maintains patient charts, and schedule appointments. 5. Ability to use calculator, computer and copy machine. 6. Phlebotomy certification preferred. 7. Current CPR certification. 8. Current negative TB skin test. 9. Pediatric experience preferred. 10. Bi-lingual (Spanish)preferred. 11. Current California driver's license and proof of auto insurance required.
