7:00 AM-3:00 PM 3:00 PM-11:00 PM 11:00 PM-7:00 AM Legacy of Love Residence is a newly licensed, six-bed Congregate Living Health Facility in Fontana, California. We provide individualized nursing care in a comfortable, home-like environment. We are seeking dependable, compassionate Licensed Vocational Nurses to join our care team. This is an excellent opportunity for an LVN who enjoys providing personalized care in a smaller setting rather than a large skilled nursing facility. Responsibilities Provide direct nursing care according to each resident's plan of care Administer medications and treatments as ordered by the physician Perform and document nursing assessments and observations Monitor residents for changes in condition and notify the appropriate provider when necessary Provide G-tube care, medication administration, feeding support and related documentation Provide tracheostomy care and suctioning when applicable Perform wound care and skin assessments as ordered Obtain and document vital signs, intake/output and other clinical information Assist with emergency response and follow facility emergency procedures Complete accurate and timely nursing documentation Communicate with physicians, pharmacies, families, home health, hospice and other members of the healthcare team as appropriate Supervise and provide direction to CNAs and other unlicensed care staff Ensure medications, supplies and treatment equipment are properly maintained Follow infection-control, medication-management and resident-safety policies Participate in shift report and communicate important resident information to the oncoming nurse Maintain a clean, organized and safe resident-care environment Qualifications Current and active California LVN license Current BLS/CPR certification Previous LVN experience preferred Experience with G-tube, tracheostomy, wound care, nonambulatory or total-care residents strongly preferred CLHF, subacute, skilled nursing, home health, hospice or long-term care experience is a plus Must successfully complete required background clearance, health screening and TB requirements Strong assessment, documentation and communication skills Reliable, punctual and able to work independently Comfortable working in a small residential healthcare setting Compassionate and respectful when providing care to residents with complex medical needs Compensation $32.00

00 per hour , based on experience, clinical skills and shift availability. Applicants with strong tracheostomy, G-tube, wound-care or subacute experience may qualify for the higher end of the pay range.
