Position Summary:

This position is responsible for providing care to meet the psychological needs of client through providing indirect services in a manner consistent with Rehabilitation and Recovery Principles. Performs comprehensive screening to clients as well as provident education. individual will initiate contacts with a variety of community resources; and implement linkages and discharge planning for clients in the Exodus Recovery Urgent Care Center.

MINIMUM QUALIFICATIONS

Current CA LVN, LPT, license in good standing. Three years' clinical practice in psychiatric care with strong emphasis working with the severely mentally ill and dually diagnosed clients. Experience with case management with exposure to populations of Chemical dependency and mental health and working within interdisciplinary team and outside community agencies is highly desired. Strong knowledge of community resources. Strong verbal and written communication skills Ability to work independently and shows mature judgment Commensurate with experience.
