In this role, you will provide high-quality nursing care to patients in various locations, ensuring safety, comfort, and effective treatment. Your skills will support patients with complex needs, including those requiring specialized care such as dementia, hospice, or acute medical conditions. This position offers an exciting opportunity to travel and collaborate with multidisciplinary teams while delivering compassionate patient-centered care. This position is paid per visit (~45min patient care, 15min documentation on the EHR $40-$45 based on experience and performance). Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to determine individual care needs. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Manage airway devices such as ventilators and provide respiratory support in line with clinical guidelines. Document all patient interactions and treatments using Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and confidentiality policies. Assist with diagnostic evaluations, specimen collection, and processing to support accurate medical diagnoses. Coordinate care plans with physicians, case managers, and other healthcare professionals for optimal patient outcomes. Provide education and support to patients and families on health management, medication administration, and post-discharge care instructions. Skills Proven experience working as an LPN in diverse settings such as hospitals, skilled nursing facilities, assisted living communities, or home care environments. Strong knowledge of medical terminology, physiology, and anatomy relevant to acute and long-term care. Proficiency in EMR/EHR systems including Epic, Cerner, or eClinicalWorks for documentation and patient record management. Skilled in IV infusion therapy, catheterization, tube feeding, and basic life support (BLS). Experience working with individuals with developmental disabilities, dementia (including Alzheimer's and memory care), or those requiring hospice or palliative care. Ability to perform triage assessments in urgent or emergency situations; familiarity with emergency medicine protocols is a plus. Knowledge of
