Summary As an LVN, you will be integral to delivering compassionate home health care to our patients, ensuring their comfort and well-being. Your core skills in patient care, senior care, and adherence to HIPAA regulations will guide your daily responsibilities. With premium skills in patient observation and assisting with activities of daily living (ADLs) to make a meaningful impact in the lives of those we serve in a home and community care setting. Responsibilities Provide compassionate home health care to patients, ensuring their comfort and safety. Monitor and document patient health status, reporting any changes to the healthcare team. Maintain compliance with HIPAA regulations and uphold patient confidentiality. Collaborate with families and other caregivers to create personalized care plans.

$35.00 - $40.00 per hour
