In this vital role, you will provide high-quality nursing care directly in patients' homes and community settings, ensuring personalized treatment plans are executed with precision and empathy. Your expertise will support individuals across a range of needs, including long-term care, acute medical conditions, and specialized populations such as seniors and individuals with disabilities. This position offers an exciting opportunity to make a meaningful difference by delivering expert nursing services in diverse environments, promoting health, independence, and well-being. Duties Conduct weekly patient visits and follow-up assessments in assisted living facilities and community settings Conduct comprehensive patient assessments, vital signs monitoring, and physical examinations in home and community settings. Administer medications, injections, IV infusions, and perform catheterizations following strict protocols and safety standards. Manage complex cases involving dementia care, Alzheimer's care, memory care, hospice support, and acute pain management. Coordinate patient care plans through detailed documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic or Cerner. Provide health coaching, education on disease management, medication adherence, and lifestyle modifications tailored to individual needs. Assist with diagnostic evaluations including specimen collection/processing and medical imaging preparation when necessary. Collaborate with multidisciplinary teams to ensure seamless discharge planning, case management, and continuity of care across various healthcare settings like, assisted living facilities, and outpatient clinics. Qualifications Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation. Experience working with individuals with developmental disabilities or behavioral health needs preferred. Proficiency in IV insertion, medication administration including injections and venipuncture techniques. Ability to perform physical examinations, vital signs assessment, specimen collection & processing while maintaining infection control standards. Excellent communication skills for patient education and interdisciplinary collaboration; understanding of HIPAA regulations essential. RN

• Home Health Registered Nurse (Per Diem) Responsibilities include: Open new home health cases and complete Start of Care (SOC) assessments Perform comprehensive patient evaluations, nursing assessments, and care planning Manage patient discharges, recertifications, and supervisory visits Coordinate discharge planning and continuity of care with physicians and multidisciplinary teams Oversee skilled nursing services and ensure compliance with home health regulations and standards Provide education and support to patients and families regarding treatment plans and ongoing care Document all patient care accurately and timely within EMR/EHR systems

Qualifications:

Current California LVN or RN license in good standing Home health experience preferred Strong clinical assessment and documentation skills Excellent communication and organizational abilities Ability to work independently and manage patient caseload effectively CPR/BLS certification required Join a supportive team dedicated to delivering high-quality, compassionate patient care in the home health setting.
