As an LVN, you will provide End of life Hospice Care, and skilled nursing care in a home health setting , working under the supervision of a Registered Nurse or physician. This role requires strong clinical skills, independence, and a compassionate approach to patient care.Key Responsibilities:
Perform comprehensive patient assessments and monitor ongoing progress. Develop, implement, and follow individualized care plans. Administer medications, including injections and IV therapy. Provide wound care , including dressing changes and monitoring for infections. Educate patients and their families about their conditions, medications, and care plans. Accurately document patient care, assessments, and progress notes. Observe and report changes in patient condition to supervising clinicians. Collaborate with interdisciplinary teams to ensure quality care.Qualifications:
Active LVN license (California). IV Certification (required). Valid BLS Certification. Minimum 1 year of nursing experience. Home Health or Hospice experience preferred. Strong skills in medication administration, wound care, and patient assessment. Excellent communication and interpersonal skills. Ability to work independently in a home health environment. Ability to lift up to 50 lbs. as needed. Valid Driver's License and reliable transportation.What We Offer:
