Responsibilities include implementing the Plan of Care, providing treatments, educating patients and caregivers, documenting clinical services, and collaborating with the interdisciplinary team to achieve positive patient outcomes. What You'll Do Provide skilled nursing care in accordance with physician orders and the established Plan of Care. Implement nursing interventions under the supervision of the Registered Nurse. Perform treatments and procedures within the LVN scope of practice. Educate patients and caregivers regarding diagnoses, treatments, medications, and the Plan of Care. Observe, assess, and promptly report changes in patient condition to the supervising RN. Complete accurate and timely clinical documentation in accordance with agency policies. Coordinate with the interdisciplinary team and communicate identified needs for additional services, including PT, OT, ST, MSW, HHA, and Dietitian. Utilize equipment and clinical supplies safely and efficiently. Participate in case conferences, staff meetings, and ongoing professional development. Perform additional patient care duties as assigned by the Registered Nurse. QualificationsRequired Current California Licensed Vocational Nurse (LVN) License Graduate of an accredited Vocational Nursing program Current CPR/BLS Certification Valid California Driver's License Reliable transportation with current auto insurance Strong communication, organization, and documentation skills Ability to work independently while collaborating with the clinical team Ability to travel throughout assigned service areas Preferred Minimum of 3 years of nursing experience At least 1 year of Home Health, Community Health, or Medical-Surgical nursing experience Experience using WellSky or other Home Health EMR systems IV Certification Bilingual is a plus Required Credentials California LVN License Driver's License Auto Insurance CPR/BLS Certification TB Test or Chest X-Ray Physical Examination Physical Requirements Ability to safely assist patients with mobility and daily care needs. Ability to frequently stand, walk, bend, kneel, and lift clinical supplies or assist patients as needed. Ability to travel between patient homes throughout assigned territories.
