LVN - Skilled Nursing Facility (SNF) We are seeking a qualified Licensed Vocational Nurse (LVN) to provide safe, compassionate, and high-quality nursing care to residents in a skilled nursing facility. The LVN will work under the supervision of the Registered Nurse and Director of Nursing.

Minimum 2 years of SNF and acute care experience within the last 3 years

Lake Isabella, CA 93240 - Kern County Key Responsibilities Assess residents' conditions and nursing needs and implement appropriate care. Administer medications safely according to the 8 medication rights . Monitor and manage pain and provide education to residents and families. Document resident conditions, care plans, progress notes, and changes in condition. Perform and interpret approved point-of-care/waived testing. Communicate changes in residents' conditions to physicians and follow up on orders. Qualifications At least 2 years of SNF and acute care experience within the last 3 years. Graduate of an accredited LVN program . Current California LVN license . Current BLS certification . Ability to read and communicate effectively in English. Basic computer skills. EHR experience preferred. Additional language skills are a plus. Ideal Candidate A compassionate, dependable, and organized LVN who can work independently, effectively supervise CNAs, communicate with residents and families, and provide safe, high-quality care in an SNF environment.
