Job Overview We are seeking a dedicated and compassionate LVN - Licensed Vocational Nurse to join our healthcare team. The LVN will provide high-quality nursing care to diverse patient populations across various settings, including long-term care, assisted living, hospitals, and community health environments. This role involves administering medications, performing patient assessments, and supporting care plans under the supervision of registered nurses and physicians. The ideal candidate will possess a strong foundation in patient care, demonstrate excellent communication skills, and have experience working with individuals with complex medical needs, including geriatrics, developmental disabilities, and acute conditions. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans. Administer medications via various routes such as injections, IV infusions, and oral medications while adhering to safety protocols and HIPAA regulations. Assist with diagnostic evaluations, specimen collection, and processing to support accurate medical diagnoses. Manage airway care, catheterization procedures, tube feeding, and ventilator support for patients requiring specialized interventions. Provide wound care, including dressing changes and infection control practices to promote healing and prevent complications. Document all patient interactions accurately within electronic health records (EHR/EHR systems) ensuring compliance with clinical confidentiality policies. Collaborate with multidisciplinary teams to coordinate patient care in settings such as skilled nursing facilities, hospitals (including ICU and trauma centers), hospice, or home care environments. Support emergency response activities through first aid procedures and basic life support (BLS) when necessary. Educate patients and their families on health management strategies related to chronic conditions like Alzheimer's disease, dementia, or pain management. Maintain a safe environment by adhering to infection control standards and long-term care regulations.
