New leadership focused on support, structure, and clinical improvement Opportunity to directly impact wound healing and patient outcomes Collaborative team environment with strong nursing leadership support Ongoing training and growth opportunities Stable, team-oriented workplace with a focus on quality care Key Responsibilities Perform and document wound care treatments in accordance with physician orders and facility protocols Complete skin assessments , monitor changes in condition, and report concerns promptly Assist in the development and implementation of care plans related to skin integrity and wound management Administer and document treatments, dressings, and therapeutic interventions Ensure compliance with infection control protocols during all treatments Collaborate with the Director of Nursing (DON) , physicians, and interdisciplinary team to optimize resident care Maintain accurate, timely, and compliant documentation in the EHR system Monitor and maintain treatment supplies and wound care inventory Participate in QAPI initiatives related to skin integrity, pressure injury prevention, and quality outcomes Assist with admissions, skin checks, and audits as needed Educate nursing staff on basic wound care practices and prevention strategies Required Qualifications Active LVN license in California (required) Minimum 1-2 years of experience in a skilled nursing facility (SNF)/ similar clinical setting Experience with wound care, treatments, and skin management Knowledge of infection control practices and documentation standards Experience with EHR systems (PointClickCare preferred) Current BLS/CPR certification Preferred Qualifications Wound care certification or experience working with a Wound Care Nurse or Wound Care Physician Strong understanding of pressure injury staging and prevention Experience participating in survey readiness and audits Core Competencies Strong attention to detail and clinical accuracy Excellent documentation and organizational skills Ability to work independently while collaborating with a team Commitment to patient safety, infection control, and quality care Reliable, accountable, and proactive Work Environment Skilled nursing and post-acute care setting Fast-paced environment requiring strong time management and prioritization Direct resident care with potential exposure to clinical and infectious conditions Benefits Medical, dental, and vision insurance 401(k) with employer match Paid time off (PTO) Paid training and CEUs DailyPay (same-day pay option) Opportunities for career advancement Apply Today If you are a skilled LVN who takes pride in clinical excellence and hands-on patient care , we encourage you to apply.
