Responsibilities:

Conduct patient assessments and develop individualized care plans Administer medications and treatments as prescribed by the physician Monitor and report changes in patient's condition Provide education and support to patients and their families Collaborate with the interdisciplinary team to ensure comprehensive care

Requirements:

Current LVN license in the state of California Minimum of 1 year of experience in a clinical setting Excellent communication and interpersonal skills Ability to work independently and as part of a team Reliable transportation and valid driver's license
