Job Description

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our skilled nursing facility team! The Vineyards Healthcare Center is a skilled nursing facility located at 76 Fenton Street, Livermore, California. It offers 24-hour Medicare-certified care in an 83-bed facility near downtown Livermore. It has received high-performing ratings in both short-term rehabilitation and long-term care from the U.S. News & World Report. RESPONSIBILITIES:
Administering medications and monitoring patient reactions Assisting with daily living activities such as bathing, grooming, and feeding Maintaining accurate patient records and documentation Collaborating with RNs and other healthcare team members to implement care plans Conducting routine physical examinations and health assessments Educating patients and families about health conditions and care plans Ensuring compliance with safety and health regulations and protocols
QUALIFICATIONS
Experience in a skilled nursing facility/long-term care/senior care is a plus Valid and active Licensed Vocational Nurse (LVN) license in the state of California. Must have an active BLS through the American Heart Association Must have an active CPR Certification Must be in good standing with all regulatory agencies and licensing boards Interest in working with the geriatric population Completion of an accredited vocational nursing program. Knowledge of the nursing process and standards of care for skilled nursing facilities Strong communication and interpersonal skills Ability to prioritize and multitask in a fast-paced environment Excellent critical thinking and problem-solving abilities Basic computer skills and proficiency in electronic health records We value teamwork and believe in creating a positive work culture that encourages personal and professional growth.
