The ideal candidate will provide comprehensive nursing services to patients in their homes, ensuring high-quality care tailored to individual needs. This role requires a strong foundation in various clinical settings, including long-term care, acute care, and specialized areas such as dementia and Alzheimer's care. The LPN will collaborate with multidisciplinary teams to deliver safe, effective, and patient-centered care, promoting health and well-being in the community. Must have previous home health experience. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health history documentation using EMR/EHR systems such as Epic, Cerner, or eClinicalWorks Administer medications and injections accurately following prescribed protocols, and catheterizations Perform wound care Provide education on disease management, medication administration, nutrition, and health coaching tailored to patient conditions Coordinate discharge planning and case management to ensure seamless transition of care across settings such as hospitals, nursing homes, assisted living facilities, or outpatient clinics Maintain detailed medical records in compliance with HIPAA regulations and documentation standards Collaborate with physicians and other healthcare professionals to develop individualized care plans for diverse patient populations including pediatrics, geriatrics, neonatal ventilator patients, or those requiring hospice or palliative care Ensure infection control practices are followed rigorously across all patient interactions This position offers an opportunity to make a meaningful difference in patients' lives through dedicated home health nursing.
