9694112-90266 Job Description Step into a rewarding contract position supporting a high school student in a transition program as a full-time Licensed Practical Nurse (LPN/LVN). Provide meaningful, one-on-one care, making a real difference in a students academic journey and daily well-being. This engaging role runs from 8:30 am to 3:30 pm, Monday through Friday, beginning as soon as possible through the end of the school year.

Key Qualifications:

Active LPN or LVN license in California Previous experience in school-based settings or pediatric nursing strongly desired Ability to work independently and manage 1:1 care responsibilities Strong communication and documentation skills Capacity to collaborate with educational and medical teams Current CPR/BLS certification

Role Responsibilities:

Deliver personalized healthcare support to a high school student with unique needs Administer medication, monitor vital signs, and perform routine medical care as prescribed Respond effectively to students health emergencies and communicate promptly with staff and guardians Assist with daily living activities and mobility as required by the individual plan Document care activities with accuracy and confidentiality Participate in team meetings and contribute to IEP or 504 planning as requested Maintain a safe and supportive environment that fosters learning and independence
