  • Are you a compassionate Licensed Practical Nurse (LPN) looking for an opportunity to positively impact the lives of adults in a long-term care environment?
We are seeking a dedicated nurse to join our team in Morgan Hill, California, providing exceptional care and support to individuals with intellectual and developmental disabilities (IDD) in a structured residential setting. In this role, you'll combine your clinical expertise with behavioral support and resident-centered care to help create a safe, therapeutic, and supportive environment.
  • What You'll Do
  • As an LPN, you will: + Deliver quality nursing care within your scope of practice + Provide general psychiatric and long-term care nursing services + Support and implement individualized treatment and behavior plans + Monitor resident health status and communicate concerns appropriately + Participate in interdisciplinary care planning meetings + Supervise DSPs and CNAs when functioning as lead staff + Provide direct resident care and therapeutic support throughout assigned shifts + Maintain accurate documentation using paper charting systems + Follow facility protocols and mandated reporter guidelines + Coordinate patient care and contribute to positive resident outcomes •Schedule & Assignment Details•+ •

  • Required

The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision, as well as critical thinking and performance ability in the coordination of patient care. Additionally, this individual performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.
  • Minimum Requirements:
