About Thalpos Thalpos is the coordination layer behind the
CMS GUIDE
Model, Medicare's dementia care program. We partner with GUIDE Participant Organizations and deploy a credentialed network of Care Companions and Care Concierges who deliver in-home respite care, home safety assessments, and telehealth support to families living with dementia. We support patients across multiple states, and we are opening new states this year. Our network is the product. If the right caregiver does not show up at the right home on the right day, nothing else we do matters. The Role The Care Network Experience LVN owns our field network. You will recruit, credential, and retain the Care Companions and Care Concierges who do the work in people's homes. You will carry real recruitment targets, stand up supply in new markets before we go live in them, and be the person our field team actually trusts and calls. When a visit goes sideways or a family raises a concern, you triage it with clinical judgment rather than a script. This role sits directly with the Founder and CEO. You will help decide which markets we enter, how we staff them, and what good looks like in the field. It is a build role, not a maintenance role. You will work from our Mountain View office 4 to 5 days per week. Some travel to new markets should be expected. What You'll Do Build the network Own recruitment targets for Care Companions and Care Concierges across our active states Run sourcing, screening, and credentialing end to end, including license verification, background checks, and onboarding Stand up caregiver supply in new markets ahead of launch, working with the CEO on market selection and staffing models Track pipeline, fill rates, time to placement, and retention, and report on them weekly Support the field Serve as the primary relationship owner for our Care Companions, the person they call when something is wrong Provide clinical guidance and support to field staff during visits, escalating to our supervising RN when a situation requires RN level assessment Triage issues raised by families about visits, caregiver fit, or care quality, and drive them to resolution Run check-ins, coaching conversations, and performance follow-up with field staff Build and refine the training, standards, and escalation pathways the network runs on Work with leadership Partner directly with the Founder and CEO on network strategy, expansion sequencing, and capacity planning Surface what you are seeing in the field: quality risks, retention patterns, market gaps, with a recommendation attached What You'll Bring Required Active, unencumbered California LVN license in good standing 2+ years of nursing experience with meaningful exposure to home care, home health, hospice, long term care, or memory care Experience recruiting, onboarding, supervising, or managing caregivers, aides, or field clinical staff Comfort carrying and hitting numeric targets Real skill with people on both sides of the relationship: caregivers who feel unsupported and families who are frustrated Strong documentation discipline and clear written communication Ability to work in our Mountain View office 4 to 5 days per week, with occasional travel Preferred Home care agency staffing or scheduling background Multi-state experience, or familiarity with caregiver credentialing requirements outside California Familiarity with the
CMS GUIDE
Model, Medicare, or value based care programs Startup or early stage experience, or a track record of building something from nothing Bilingual (Spanish or Tagalog a strong plus) Schedule Monday through Friday, business hours, in our Mountain View office 4-5x per week.
