This position supports providers and clinic teams through triage assistance, quality control, clinical documentation support, SBAR guidance, and backup support to staff as needed. Essential Responsibilities Provide clinical support within LVN scope of practice and in accordance with applicable laws, regulations, policies, and procedures. Support clinical triage workflows by reviewing patient concerns, gathering relevant information, escalating urgent or complex issues appropriately, and communicating with providers or leadership as needed. Serve as a resource for SBAR communication by helping staff organize concise, clinically relevant information for provider review and decision-making. Support provider-led Beta workflows by assisting with clinical documentation structure, workflow clarification, and follow-up items as directed. Perform quality control activities related to clinical documentation, patient care follow-up, task completion, and clinical workflows. Clinical support as needed, including assisting with patient-facing or floor-based tasks during periods of high volume, staffing gaps, or operational need. Assist with patient education, follow-up communication, and care coordination as appropriate and within scope. Maintain accurate, timely, and complete documentation in the medical record or electronic health record. Communicate effectively with providers, clinical staff, leadership, patients, families, pharmacies, laboratories, and other members of the care team. Identify workflow gaps, documentation issues, and opportunities for improvement, and escalate findings to the Clinical Director or appropriate leadership. Follow confidentiality, HIPAA, compliance, safety, and infection prevention standards at all times. Participate in quality improvement activities, team huddles, staff meetings, training, and Beta-related workflow discussions as required.

Essential Skills, Knowledge, and Abilities:

Knowledge of basic nursing principles, medication administration, infection control, and patient safety standards. Ability to work within LVN scope of practice and seek appropriate clinical direction when needed. Strong documentation, communication, organization, and time-management skills. Ability to use electronic health records and standard office or clinical technology. Education Requirements:

Required:

Current, active Licensed Vocational Nurse license in the state of practice; California LVN license required for California-based roles. Graduation from an accredited or state-approved vocational nursing program. Current Basic Life Support (BLS) certification, or ability to obtain within required timeframe.

Work Experience Requirements:

Required:

One or more years of LVN experience in outpatient primary care, ambulatory care, or medical group setting. Experience with clinical triage support, patient follow-up, care coordination, referrals, prior authorizations, or chronic disease management. Experience supporting quality control, workflow standardization, documentation review, or clinical process improvement. Familiarity with SBAR communication, clinical documentation templates, and electronic health record workflows. Bilingual skills, especially Spanish.
