Candidate must be able to identify patients that meet specific criteria as identified through telephonic interactions and medical record reviews. Must have good communication skills and able to manage both inbound and outbound calls in a timely manner. Have strong phone and verbal communication skills along the ability to provide concise patient education. Additionally, candidate must be able to work collaboratively with clinical teams and have basic computer, documentation and reporting skills.

Qualifications:
