Field Nurse Duties:

• Provide comprehensive nursing care to patients in their homes or other facility settings
• Conduct assessments and develop care plans for patients
• Administer medications and treatments as prescribed
• Monitor patient vital signs and assess for any changes in condition
• Educate patients and their families on proper healthcare practices and disease management
• Coordinate with other healthcare professionals to ensure continuity of care
• Document patient information accurately and timely

Skills:

• Experience in hospice and home health field nurse
• Experience with plan of care and timely documentation
• Knowledge of medical coding and documentation practices
• Familiarity with family planning methods and counseling techniques
• Excellent nursing assessment, communication, and problem-solving skills
• Ability to lift and move patients safely using proper body mechanics
