Job Summary We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our skilled nursing facility team. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to develop individualized care plans. Administer medications and injections accurately following physician orders, including IV infusion when applicable. Monitor residents' conditions closely, recognizing signs of acute pain, infection, or deterioration, and respond promptly with appropriate interventions. Assist with activities of daily living such as tube feeding, catheter care, and mobility support using equipment like Hoyer lifts. Document all patient care activities thoroughly in electronic health records (EHR/EHR systems) while maintaining strict compliance with HIPAA and confidentiality policies. Collaborate with physicians, therapists, and caregivers to coordinate care plans tailored to residents' needs in long-term care, memory care, or hospice settings. Educate residents and their families on health management topics including dementia care, medication administration, and safety protocols. Experience Valid LVN license with current state licensure required. Prior experience working in skilled nursing facilities (SNF), long-term care environments, or assisted living communities preferred. Critical care experience including ICU or emergency medicine is highly desirable. Familiarity with EMR/EHR systems such as Epic or Cerner is advantageous. Knowledge of ICD coding (ICD-9/ICD-10), medication management, and medical documentation standards essential for accurate recordkeeping. Experience working with geriatric patients or individuals with developmental disabilities enhances your ability to deliver specialized care. Additional experience in hospice care, Alzheimer's/dementia/memory care settings, or acute pain management will strengthen your application.
