Duties Conduct thorough patient assessments, vital signs monitoring, and physical examinations in various community settings Administer medications, injections, IV infusions, and perform catheterization following strict aseptic techniques Manage airway care, tube feeding, wound dressing changes, and ventilator support as needed Document all patient interactions accurately using electronic health records (EHR/EHR systems) while maintaining clinical confidentiality policies Educate patients and caregivers on health management, medication adherence, and disease processes such as dementia or chronic pain conditions Collaborate with healthcare teams to develop and implement personalized care plans aligned with long-term care regulations and best practices Perform specimen collection/processing and assist with diagnostic evaluations to support comprehensive patient care Experience Valid LVN license with current licensure in the applicable state Proven experience working in diverse healthcare environments such as skilled nursing facilities, hospitals (including ICU or trauma centers), assisted living communities, or home health settings Critical care experience including ICU or emergency medicine is highly desirable Familiarity with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate medical documentation Experience working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's or memory care is preferred Knowledge of
