Job Summary We are seeking a dynamic and compassionate Licensed Vocational Nurse (LVN) - Field Nurse to join our dedicated healthcare team. In this vital role, you will provide high-quality, personalized nursing care directly in patients' homes and community settings. Your expertise will support individuals across various stages of health, including seniors, those with chronic conditions, and individuals with disabilities. Duties Conduct comprehensive patient assessments including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or eClinicalWorks Administer medications, injections, IV infusions, and perform catheterizations following sterile technique and safety protocols Provide wound care, including dressing changes and infection control measures for patients with complex needs such as pressure ulcers or surgical wounds Manage airway and ventilator support for patients requiring acute or chronic respiratory assistance Educate patients and caregivers on disease management, medication adherence, nutrition, and self-care strategies tailored to individual needs such as Alzheimer's care or dementia management Coordinate with interdisciplinary teams for case management, diagnostic evaluation, and treatment planning while ensuring compliance with long-term care regulations and HIPAA confidentiality policies Document all patient interactions accurately in EMR/EHR systems like Cerner or Home & Community Care platforms; ensure medical records are complete and compliant with privacy standards Requirements Valid LVN license in the state of practice with current CPR/BLS certification Proven experience working with diverse populations including seniors, individuals with developmental disabilities, or those requiring hospice or palliative care Critical care experience preferred, including ICU or Level I Trauma Center exposure; hospital medicine or emergency room experience is highly valued Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards Proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks; experience managing electronic health records efficiently Strong knowledge of physiology, anatomy, pharmacology related to medication administration and pain management techniques Ability to work independently in the field while maintaining excellent communication skills for client interaction and caregiver education Experience working with individuals with disabilities or special needs is a plus; familiarity with dementia care, memory care or geriatric patient needs is desirable Join us in delivering compassionate nursing care that empowers individuals to live healthier lives!
