Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems. Administer medications accurately while adhering to HIPAA regulations and sterile techniques. Document all patient interactions meticulously, ensuring compliance with medical terminology standards such as

ICD-9, ICD-10

coding, CPT coding, and accurate medical records management. Provide specialized dementia and Alzheimer's care, including memory care strategies and discharge planning for post-acute or hospice patients. Support patients through physical examinations, specimen collection/processing, suture procedures, and infection control protocols. Educate patients and families on health coaching topics such as nutrition, medication management, family planning, or post-discharge instructions. Experience (Not required but preferred) Proven LVN experience in diverse clinical settings such as hospitals, nursing homes, assisted living facilities, or outpatient clinics. Familiarity with EMR/EHR systems Knowledge of advanced patient assessment methods for working with individuals with disabilities or complex medical conditions such as infectious disease care or pain management.
