• Job Overview:

• We are seeking a dedicated Field Nurse to join our healthcare team. The Field Nurse will provide high-quality patient care in various settings, ensuring the well-being of individuals under their care.

• Duties:

• Perform home visits to monitor patient condition and educate on medication and disease process management
• Conduct patient assessments and develop individualized care plans
• Administer medications and treatments according to physician orders
• Monitor and record vital signs, symptoms, and treatment outcomes
• Collaborate with healthcare professionals for comprehensive patient care
• Assist in discharge planning and patient education
• Utilize physiology knowledge to assess and address patient needs
• Provide specialized care in areas such as wound care, foley catheter care, and ostomy care

• Experience:

• Prior experience in patient care, preferably in a home health or hospice setting
• Proficiency in EMR documentation preferably with Devero, Hospice MD, Synergy or similar software.
• Familiarity with sterile processing techniques
• Strong understanding of vital signs monitoring
