Position Summary The LVN/LPN will provide skilled nursing services to patients in their homes under the supervision of the Registered Nurse and clinical leadership team. This position offers autonomy, flexibility, and the opportunity to build meaningful relationships with patients and families. Responsibilities Perform skilled nursing visits in patients' homes Monitor patient condition and report significant changes Administer medications and treatments as ordered Provide patient and caregiver education Complete accurate and timely documentation Coordinate care with physicians, therapists, case managers, and interdisciplinary team members Follow agency policies and Medicare/state regulations Maintain professionalism and compassionate patient care at all times Qualifications Active California LVN/LPN license in good standing Current BLS/CPR certification Home health experience preferred, but not required Excellent communication and organizational skills Reliable transportation, valid driver's license, and auto insurance Ability to work independently and manage time efficiently Compensation $70
