RN/LVN MDS

The MDS Coordinator plays a critical role in ensuring the accurate assessment and documentation of patient care within a healthcare facility. This position is responsible for coordinating the Minimum Data Set (MDS) assessments, ensuring compliance with federal and state regulations, and maintaining high standards of medical documentation. The ideal candidate will have at least one year of experience as MDS Coordinator in a SNF setting , medical records management, and familiarity with Level II Trauma Center protocols. Duties Coordinate and oversee the completion of MDS assessments for patients in accordance with regulatory requirements. Review medical documentation and records to ensure accuracy and compliance with Medicare standards. Collaborate with interdisciplinary teams to gather necessary information for MDS submissions. Conduct documentation reviews to identify areas for improvement and ensure adherence to HEDIS measures. Maintain up-to-date knowledge of DRG classifications and their implications on patient care and reimbursement. Serve as a resource for staff regarding MDS-related inquiries and training needs. Monitor compliance with established standards and implement corrective actions as needed. Requirements 1-year or more proven experience as MDS coordinator, particularly with resident assesments. Familiarity with Level II Trauma Center operations is preferred. Strong understanding of DRG, HEDIS, and Medicare documentation standards. Excellent organizational skills with attention to detail to ensure accurate assessment completion. Ability to work collaboratively within a multidisciplinary team environment. Strong communication skills, both verbal and written, to effectively convey information to staff and stakeholders. Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare. This position is essential for maintaining quality patient care through effective coordination of assessments and adherence to regulatory requirements.
